A request for Name Change must be submitted in writing and addressed to:

Graduation – “Change of Name”
c/o Tirisa Fonmanu
Student Administrative Services (SAS)
Laucala Campus
Suva

Complete the "Request for Change to Personal Details form"  and it should be accompanied by the relevant legal documentation confirming the change.

All required changes must be provided to SAS a month before the graduation ceremony. No further changes will be allowed after the certificate has been awarded.
