Documents to Submit and Upload to Moodle
1. The
applicant must use the Faculty Research Proposal form for grant proposals, or
Progress Report forms to report on the current status of report. Note that the last
page must contain only the signed declarations of the Principal Investigator, and/or
the Research Group Leader, and/or the Head of School, since the page must be
scanned and submitted together with the rest of the form as a single PDF.
2. The
applicant must submit the Screening Questionnaire for Human Ethics,
downloadable from the USP Research Office website.
3. If
the research involves animals, particularly in the areas of agriculture, and
terrestrial and marine biodiversity (e.g., pigs, sheep and other mammals, as
well as birds, reptiles, amphibians and fish) then the applicant must also
submit the Animal Ethics Application Form, downloadable from the USP Research
Office website.
All the above documents must be
uploaded as a SINGLE PDF to the Moodle-based FSTE Admin Portal.
All FSTE staff and postgraduate
can access this portal.
